your Medicare card, a copy of your state health care program
identification card, or a letter stating that you are in financial need.

sponsored membership you will receive all the benefits and services of
MedicAlert(r) membership. This includes any one of the basic emblems
available, i.e., stainless steel bracelet, necklace, dog tag or sports
band for children. Note that all MedicAlert(r) emblems are customized
and comes in varying lengths.
